experienced physicians, nurse practitioners, and medical
staff who can assist you with managing all of your
healthcare needs, including:

  • Digital X-Ray Services and Outside Imaging Services

  • Vital Health Profile (formerly Health Risk Assessment)

How To Book Appointment Flyer


How to Schedule an Appointment:

  1. www.myhealthonsite.com/patient-access, Once on the Patient Portal page, enter 
    Username
    and Password
  2. Click Appointments on the left navigation bar, click New Appointment (or at the 
    top of main page, s
    elect Search For Appointment)
  3. Choose a Reason for Visit and then Location (All, Port Charlotte, or Punta Gorda)
  4. Select Provider and then select Show Availabilities
  5. Select desired Date and Time (also choose a future date) then select Go
  6. Patient Details page confirm information and appointment details, click Next and 
    go Visit Detail page
  7. Select Yes or No, enter any information you would like to provide to the Provider, select 
    Pay by Cash/Not Applicable,
    and select Next
  8. Click Voice or Text for verification, enter security code and then Book Now

There are two employee health center locations - Login to My Health Onsite to view doctor/staff and appointment availability.

Bob Pryor Employee Health Center

South County Employee Health Center

514 E. Grace Street, Punta Gorda


Please note - South County Employee Health Center does not do:

  • Occupational testing/Urine Drug Screening

  • X-ray

  • Workers’ Compensation related services

CALL CENTER NUMBER 
Call Center Support Team: 941-800-2005 

Hours of Operation: The Call Center is open to take appointments Monday–Friday from 8am to 5pm. Face-to-Face, Televisit (via Video), and Telephonic (via Telephone) appointments will continue to be available. If you need to reach the Support Team after hours, please call 941-800-2005, you will be routed to our nurse line for ongoing patient services.
