The Wellness Program is focused on the Body Mass Index and weight loss.
For employees that meet the selected weight targets or participate in a wellness program at the guidance of the Employee Health Center, the County will provide each enrolled employee (and one dependent spouse if covered) on the County’s medical plan with up to $500 in a Health Reimbursement Account (HRA).
If the employee/eligible spouse’s BMI is greater than 25 then use the Weigh-In Chart to calculate your benchmarks needed to earn all or part of the incentive.
Enter your weight at the time of your PHRA weigh-in and hit calculate to see the amount of your incentive based on percentage of weight lost.
